800-620-5010; www.directbenefits.com. Mimimum group size is 2; ideal for mom-pop and family-owned businesses. Has two plans, Denali Vision and Spirit Vision. Unlimited yearly benefit with a $10 to $25 copay. Eye exams are covered 100 percent if provider is in the EyeMed Network or Avesis network.
